Safety Management: Task Analysis
When an incident, or problem, appears to have resulted from a human error during the execution of a task, or procedure, a task analysis should be performed. The objective of a task analysis is to determine how a task was actually performed, compare that to how it should have been performed, and identify corrective actions that will increase the likelihood that it will be performed correctly in the future. This module describes the steps involved and how to perform a task analysis.

Key Questions
What is a task analysis?
A task analysis is an evaluation of people doing a task, the environments in which they work, the physical and mental actions they perform, and the resources necessary to complete the task.
What environmental factors should be evaluated?
Both the physical and organizational environments should be evaluated. For example, the amount of light or noise, temperature, or time of day are physical factors and the company culture, time pressures, and crew dynamics are organizational factors that affect the performance of a task.
What are some example resources?
A resource is anything needed to successfully complete a task. This includes procedures, manuals, tools, PPE, or even the number of people needed.
What are the steps of a task analysis?
To perform a task analysis, state the purpose of the analysis, determine how the task should be done and compare that to how it is actually done, and then identify corrective actions to address the deficiencies.
How do you get information about how a task was actually done?
Investigation methods will vary. Interview people involved and have them explain what happened. If possible have them reenact what was done.

When analyzing people requirements, there are a number of physical, physiological, and psychological factors to consider. There can be physical traits, such as size, age, or strength that are required for an individual performing the task. The number of individuals required to perform the task is another important consideration. There are a number of physiological factors that can affect a person’s performance of a task. For example, a worker may be sick, fatigued, or addicted to drugs or alcohol. Finally, but most importantly, a person must have the knowledge, skills, and personality necessary for the task. Effective training and staffing is critical. Someone’s attitude and emotional state also affects their ability to complete a task.
